Obtaining CCRL Proficiency Sample Program Request Forms

To participate in the CCRL proficiency sample programs contact us for the appropriate request form. Return the request form along with a non refundable check (payable to ASTM) equal to the program fee plus shipping charges. Program fees for the portland cement and concrete programs can vary depending on the time of the year the program is entered. Shipping charges vary and will be calculated when you contact CCRL for the appropriate request form. (Appropriate taxes for Canada, Maryland, and Pennsylvania will be added.)
