* The shipping address you provide below will be used for shipping proficiency samples to your laboratory

* All of our domestic shipping is done via FedEx. They often require a signature for delivery.

* If your laboratory has participated in our programs in the past, please provide your CCRL lab number in the remarks.

* The Proficiency Sample Program is a pre-pay program. If the program you are trying to enroll in is closed, we will add your request to our wait list and generate your invoice once the next year’s prices are available.

* Once we receive your completed form, we will have an invoice generated for your enrollment as quickly as possible. Your laboratory is not considered enrolled until payment in full is received.

* All of our international shipping is done via FedEx Express or UPS Express. You must provide your courier and account number prior to CCRL generating an invoice for your enrollment. For additional shipping guidelines, please contact ccrl@astm.org.

* If your laboratory has participated in our programs in the past, please provide your CCRL lab number in the remarks.
